Seniors' Community Connector - Social Prescribing Program - Surrey

Connects seniors to non-clinical services to help improve their health and wellness.
Social prescribing is a way for local agencies to refer people to a Community Connector. Community Connectors give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community resources and services for practical and emotional support.

Community Connectors will collaborate to develop wellness goals and provide ongoing support tailored to individual needs, helping to achieve desired outcomes. They can provide information, advice, guidance, and support, as well as assist with accessing a range of services that meet individual needs.

Services include:
  • Support to make positive changes in your life
  • Someone to talk to about how you are feeling
  • Help with housing, benefits and financial issues
  • Physical activity and recreation
  • Art, music and creative activities
  • Access specialist services and support
  • Support to problem solve and work through challenges in your life
Social Prescribing can help seniors who:
  • Feel lonely or isolated
  • Are caring for someone
  • Are feeling low, stressed or down
  • Want to get more active
  • Want to get involved in the local community
  • Want to make a lifestyle changes for improved health and wellbeing
Referrals to the Social Prescribing program can be made by a GP, Nurse Practitioner, Home Health, Older Adult Mental Health, Peace Arch Hospital discharge and pre-surgical admission teams, the Specialized Seniors Clinic, or the Primary Care Network team. Self-referrals are also accepted
