Information on how to report a child's immunizations to the public health unit.
Why should I report my child's immunizations? It’s important for public health to have an up-to-date record of your child’s immunizations so they can:
  • Respond effectively to disease outbreaks in childcare facilities and schools
  • Recommend immunizations a child may be missing
  • Provide the best care for a child through public health programs

When should I report a child's immunization record?
  • If the child is immunized outside of the Fraser Health region (e.g. Vancouver, Richmond, or out of province).
  • If the child is immunized by a pharmacist.
  • Before a child starts kindergarten
There are four ways to report your child’s immunization record:
  • In person; Drop off a photocopy of the child’s immunization record to the public health unit.
  • Call; Report immunizations over the phone to the public health unit. If directed to voice mail, leave the best number to be reached during the day and the call will be returned.
  • Fax or mail; Send a copy of the child’s immunization record by fax or mail to the public health unit.
  • By email; Scan, take a photo or type the child’s record and email to: reportimmunizations@fraserhealth.ca
