Seniors' Community Connector - Social Prescribing Program - Surrey

Provides support for seniors to access non-clinical, community-based services. Serves Surrey.
Social Prescribing is an approach to offering non-medical support that enables older adults to identify, set and achieve personal health and wellness goals. A Community Connector helps to find appropriate local programs and services and provides regular check-ins to ensure clients are on track. PICS has a Community Connector supporting older adults in Surrey.
Eligibility:
  • Residing in Surrey
  • 65 years of age or older
  • Experiencing, or at risk of: frailty; social isolation and/or loneliness; barriers to access community-based resources; frequent trips to primary care or ER.

Referral options:

  • Health professional referral
  • Community service organization referral
  • Self-referral
  • Family physician or nurse practitioner referral

Details: Referral is required. Referrals can be made by healthcare professionals, community organizations, or through self-referral. Once a referral is received, a Community Connector will follow up with the client to complete an intake assessment and connect them to appropriate services based on their needs.
