Home and Community Care offers a range of health care and support services at home and in the community.

Services

Fraser Health offers both short-and long-term health care services at home. Short-term support helps a patient recover from a hospital procedure or condition. Long-term programs provide ongoing care that will give patients the independence they need to remain at home comfortably. A community health professional completes an assessment for the following services (to determine which may be provided_:
  • Advance care planning
  • Nutrition and dietitian services
  • Rehabilitation therapy
  • Community health nursing
  • Caregiver support clinicians: Social workers who provide a wide range of services to caregivers caring for current Home Health clients. Services include system navigation, advocacy, support for burnt/out stress, access to community resources, etc.
  • Long Term Care and Assisted Living Overnight Respite Service: A short stay in Long Term Care for community clients providing caregiver rest and relief from the emotional and physical demands of caregiving duties.
  • Day program for older adults: Program provides weekly social connection, fun activities, exercise and health checks for older adults who have health challenges and want to keep living as independently in the community. While also providing respite for caregiver.
  • Seniors community connector: Access to community services and activities to help you stay well.
  • Social work services: Access to programs, services and resources to help with isolation.
  • Home support services: Personal care supports and daily home-based activities.
  • Home care nursing services: Wound or skin care, managing medications, intravenous therapy and additional home supports.
  • Home oxygen program
  • Palliative care
  • Palliative care information for physicians


New clients can contact the Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line) by calling 1-855-412-2121 (open 7:00 a.m. – 9:00 p.m., 365 days a year) or they can book a telephone appointment by filling out their online form. Current clients can contact their Home Health location.
