Diabetes Education Program - Northern Interior Rural

Provided by Northern Health

Provides outpatient education and support to people living with diabetes, their family members and the community. Topic areas include healthy eating, blood glucose testing, exercise, and staying healthy while living with diabetes
Programs Include:
  • General education: Foundations for diabetes self-management including basic pathophysiology, blood glucose management, nutrition therapy and prevention of complications.
  • Insulin initiation: Physician prescription required to initiate insulin.
  • Individual appointments: Available for patients not suitable for group participation due to e.g., vision, hearing, frailty, cognitive or behaviour impairment, language barriers, complex medical management and individuals with Type 1 diabetes.
  • Advanced insulin management: Assisting patient in achieving optimal glycemic control through insulin dose adjustments.
  • Gestational diabetes: Regular follow-up, CBGM education and nutritional support throughout pregnancy.
  • Type 1 diabetes: Regular follow-up, education, insulin management, insulin pump management and nutritional support.
  • Diabetes in pregnancy: Pre-conception counseling and intensive diabetes management for women with preexisting Type 1 or Type 2 diabetes.

    Diabetes centre specialist: Specialist referral is recommended if any of the following are present:
  • A1c greater than 9.0%.
  • Significant diabetes complications.
  • 3 or more anti-hyperglycemic medications or MDI (multiple doses of insulin).
