Surrey - North Delta

This community-based cardiac rehabilitation and prevention program aims to identify people with cardiovascular disease or those requiring risk factor modification. The program counsels participants on an evidence-based regimen of medical therapy and lifestyle optimization. Sessions run in January, April, and September. The program includes the following:

• An 8-week series comprised of weekly 2-hour group sessions (1-hour of education, 1-hour of supervised exercise)
  
• Education topics include intro to heart disease, risk factor modification, blood pressure management, stress management + mindfulness, dietary sodium, fats + fibre, into to diabetes, mindful eating, and medication review with a community pharmacist.
  
• The program is divided into three parts: Supervised Exercise Class - 2 morning sessions per week, Nutrition Class - 1 morning session per week, Life Skills Class - 1 morning session per week
  

The life skills and nutrition classes are offered alternatively on the same mornings you attend the exercise classes.

The clinic also sees patients who have undergone other cardiac surgical procedures i.e. valve repair/replacement, pacemaker insertion, post ablation procedures and those who have a diagnosis of congestive heart failure. Cardiac rehabilitation is considered best practice for the recovering cardiac patient. An interpreter is available upon request for initial appointment

This program is by referral only.

Cost for Program:
There is a fee of $317 to the patient for this program. If the patient's medical insurance policy has extended benefits coverage, the receipt of payment may be submitted to the insurer under 'physiotherapy' benefits. If this fee is a barrier to accessing this service, please call the program directly and ask to speak to a social worker to determine if a financial subsidy would be applicable.
